I'm guessing the "5" on the sleeve is in honor of Lou Boudreau, who died 8/10/2001.

Unfortunately, #26 in 2001 was Bob Wickman, and that sure as heck ain't Bob Wickman.

I'm pretty sure it's 26, too. In '99, 26 was Jesse Levis, and in '98 it was Jeff Manto (who also wore #12 at some point).

I think Bob nailed it in 5. Long sleeves back up the October date, everything else (Cordova, #46, McLemore's patch, Boudreau's #5) matches.
